Total testosterone measures the overall amount of testosterone circulating in your blood, including both bound and unbound forms.
About 97-99% of testosterone is bound to proteins, primarily sex hormone-binding globulin (SHBG) and albumin. The remaining 1-3% circulates freely and is known as free testosterone.
Total testosterone is the most commonly ordered test for evaluating androgen status. It provides a starting point for assessing testosterone levels, though free or bioavailable testosterone may be needed for a complete picture when SHBG levels are abnormal.
Where Does Testosterone, Total Come From?
In men, about 95% of testosterone is produced in the Leydig cells of the testes. In women, testosterone is produced in smaller amounts by the ovaries. Both sexes also produce testosterone in the adrenal glands.
