What is Omega-3 Total?

Total omega-3, often measured as the Omega-3 Index, represents the percentage of omega-3 fatty acids (EPA and DHA) in your red blood cell membranes relative to all fatty acids present.

Because red blood cells turn over slowly (over about 120 days), this measurement reflects your long-term omega-3 status from diet and supplements over the past 3-4 months, similar to how HbA1c reflects long-term blood sugar levels.

An Omega-3 Index of 8-12% is generally considered optimal, while levels below 4% indicate deficiency. Most Americans have an index around 4-5%, which falls in the higher-risk zone.

Where Does Omega-3 Total Come From?

Your body cannot efficiently produce EPA and DHA, so these must come primarily from dietary sources such as fatty fish (salmon, mackerel, sardines, herring), fish oil supplements, or algae-based supplements. While the plant-based omega-3 ALA (found in flaxseed and walnuts) can be converted to EPA and DHA, this conversion is very limited in humans.
